Dental Record Keeping Guidelines: A UK Practice Guide

Dental record keeping guidelines in the UK come from three main places. The General Dental Council requires every registrant to make and keep contemporaneous, complete and accurate records. NHS England's consensus standards list what should be recorded at new patient, recall and urgent appointments. And the College of General Dentistry's good practice guidelines cover the detail. Together they expect records that are written at the time, easy for others to read, never altered without trace, and kept for the right length of time.

Poor records are one of the most common weak points in complaints, audits and fitness to practise cases. Good ones protect patients, support the next clinician who treats them, and protect the practice. This guide sets out what the rules say, what a complete record contains, how to correct it properly, and what to look for in electronic records. The standards quoted were checked in September 2026.

Who sets the rules on dental record keeping?

• The General Dental Council (GDC). Its Standards for the Dental Team apply to every registered dental professional in the UK.
• NHS England. Its Dental Record Keeping Standards: a consensus approach, published in October 2019, sets out what to record for three types of appointment.
• The College of General Dentistry. Its Clinical Examination and Record-Keeping: Good Practice Guidelines is the detailed reference, now in its third edition.
• Data protection law. UK GDPR and the Data Protection Act 2018 govern how records are stored, shared and accessed.
• The Care Quality Commission (CQC). In England, inspectors look at records as evidence of safe and effective care.

What the GDC standards say about records

GDC standard 4.1 says you must make and keep contemporaneous, complete and accurate patient records. The detail sits in four points:

• 4.1.1: keep complete and accurate records, including an up-to-date medical history, each time you treat a patient. Radiographs, consent forms, photographs, models, lab prescriptions and referral letters all form part of the record.
• 4.1.2: record as much detail as possible about your discussions with patients, including evidence that valid consent was obtained.
• 4.1.3: follow current law on patient information, and follow national advice on keeping, storing and disposing of records.
• 4.1.4: make sure records are clear, legible, accurate and can be readily understood by others, and record the name or initials of the treating clinician.

"Contemporaneous" is the key word. Notes should be written at the time, or as soon as possible afterwards. A note written days later from memory is weaker evidence, and it should say when it was written.

NHS England's consensus standards

NHS England's standards were agreed through a Delphi process with clinicians, regulators and patient groups. Their stated aim is "one patient, one record, one standard". They provide three templates:

• New patient examination
• Recall patient examination
• Urgent patient examination

For each appointment type, every item is graded as essential, aspirational, conditional or not required. That makes the standards a practical audit tool. You can take ten records from each clinician and check them against the essential items for that appointment type.

What a complete dental record should contain

The exact items depend on the appointment, but a complete record usually includes:

• Personal details: name, date of birth, address and contact details.
• Medical history: updated at each course of treatment, including allergies, conditions and medications.
• Reason for attendance: the patient's complaint in their own words.
• Social history: such as smoking and alcohol, where relevant to risk.
• Examination findings: extra-oral and intra-oral findings, the tooth chart and soft tissues.
• Periodontal screening: the BPE, and a pocket chart where the scores call for one. Our guide to BPE scores explains the codes.
• Radiographs: the justification, and a report of what was seen.
• Diagnosis and treatment options: including the risks and benefits discussed.
• Consent: what the patient agreed to, and a written treatment plan. Our dental treatment plan template covers what that must include.
• Treatment provided: including materials, and batch numbers where relevant.
• Recall interval: the interval chosen and why.
• Who and when: the clinician's name or initials, and the date.

How to correct a dental record

Mistakes happen. A tooth is charted on the wrong side, or a note goes on the wrong patient. The rule is simple: correct the record, but never hide what it said before.

• On paper: strike through the error with a single line so it stays readable, then write the correction with the date and your initials.
• Electronically: add a correction or addendum. The system should keep the original entry and show who changed it, when and why.
• Never delete, overwrite or back-date an entry to make it look as if it was always right.

A record that shows its corrections is more trustworthy, not less. It shows a clinician who noticed an error and fixed it openly.

How long should dental records be kept?

GDC standard 4.1.3 says you must follow national advice on keeping and disposing of records. In England, that advice is the NHS Records Management Code of Practice. Its August 2023 amendments reduced the retention period for dental clinical care records from 15 to 11 years.

Children's records follow longer rules, and different retention guidance applies in Scotland, Wales and Northern Ireland. Check the current schedule for your nation, and write your practice's retention periods into a policy. Periods are a minimum, and records involved in a complaint or claim may need to be kept for longer.

Access, sharing and data protection

Patients have the right to see their own records under UK GDPR. Requests should be handled promptly and logged. Records must also be kept confidential and shared only with a lawful basis, for example with a specialist on referral. Auditing your record keeping

Record keeping audits are one of the simplest quality improvements a practice can run. A basic cycle looks like this:

1. Choose a standard, such as the NHS England essential items for a recall examination.
2. Pull a sample of records from each clinician, for example ten each.
3. Score each record against the standard.
4. Share the results, agree changes and update templates.
5. Re-audit after three to six months.

What to look for in electronic dental records

Most practices now keep records electronically. The software should make good record keeping the easy path. Check that it:

• Stamps every entry with the author and the time automatically.
• Keeps the original when something is corrected, and shows the reason.
• Lets the tooth chart, notes, medical history and plans sit on one record.
• Keeps a log of who signed in and what they did.
• Controls who can see and change clinical information by role.
